BY CLICKING OR CHECKING “I ACCEPT”:

• (A) YOU ARE REPRESENTING THAT YOU HAVE THE POWER AND AUTHORITY TO BIND YOURSELF AND (IF YOU IDENTIFY YOUR EMPLOYER) TO BIND YOUR EMPLOYER TO THE TERMS AND CONDITIONS OF THIS AGREEMENT; AND
• (B) YOU ACKNOWLEDGE THAT YOU HAVE READ THIS AGREEMENT, UNDERSTAND IT, AND ARE CONSENTING TO BE BOUND BY THIS AGREEMENT ON BEHALF OF YOURSELF AS AN INDIVIDUAL AND (IF YOU IDENTIFY YOUR EMPLOYER) ON BEHALF OF YOUR EMPLOYER.

The identified Individual and Employer (if provided) (each of which is individually referred to herein as “you” or “your”) hereby give and grant in perpetuity to Dolbey & Company, Inc., (“Dolbey”) the irrevocable right and license to publish, reproduce, distribute, and/or otherwise use your name, your title, the video and your statements in the video (in whole or in part) relating to Dolbey products and services created on or about the Date (collectively, the “Materials”). Dolbey may use the Materials in such manner, for such purposes, and with such frequency, as Dolbey shall determine in its sole discretion, without further compensation or consideration to you and without further authorization by you. This consent and release is given without limitation upon any use of the Materials for advertising, illustration, publication or broadcast of every kind, or in trade or media, including the internet, for every purpose.
